Passport Acceptance Service

American citizens can apply for a U.S. passport through the Crowell Public Library’s Passport Acceptance Service. Passport photo service available for $15. U.S. citizens traveling by air from Canada, Mexico, the Caribbean, and Bermuda must present a U.S. passport book when entering the United States.

Effective June 1, 2009 U.S. citizens must present a valid U.S. passport book, U.S. passport card, or other travel document approved by the Department of Homeland Security, when entering the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry.

What to Bring 
  • A completed DS-11 Form, filled out in BLACK ink. 
    • Key Information Needed: 
      • Applicant's Social Security Number 
      • Applicant's parents' birth date and birth place. 
  • Proof of U.S. Citizenship: Certified Birth Certificate, Certificate of Naturalization, or previous U.S. Passport. 
    • Photocopies are NOT acceptable, all documents must be certified.
    • California and Texas Abstracts are never acceptable.
    • Children under the age of 16 renewing their passports must submit a Certified Birth Certificate as well as their previous passport. 
  • Proof of Identity      
    • Valid Driver's License, State ID Card or Previously issued U.S. passport book/card. 
      • Those with out of state identification must provide a secondary form of identification. 
  • All applicants must appear in person. 
    • Children under the age of 16 need both parents to be present or provide a notarized DS-3053 along with a photocopy of the front and back of the identification presented to the notary.
      • Form DS-3053, Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor Under Age 16, is used when only one parent can be present at the time of application (execution). It should be completed by the non-applying parent and notarized, or completed by the applying parent when written consent of the applying parent cannot be obtained. This form should be submitted with the DS-11 Form.
  • One recent color 2 in.x 2 in. Passport photo.

How do I check the status of my passport?
You can check the status of your application with the Department of State here. You will need to know the applicant's last name, date of birth and last four digits of their social security number. 

What is the difference between a Passport Book and a Passport Card?
The Passport Book can be used for international travel by air, sea or land. The Passport Card can be used when entering the United States from Canada, Mexico, the Caribbean and Bermuda at land border crossings or sea ports-of-entry. The Passport Card is not valid for international travel by air. 

How long will it take to receive my passport?
Routine passports have an expected turnaround time of 4-6 weeks. Passports can be expedited for a fee and have an expected turnaround time of 2-3 weeks. Passports needed within a shorter period of time cannot be processed at the library. 

Can I renew my passport at the library?
If the applicant is under the age of 16 or was at the time their previous passport was issued, they must apply in person. Qualifying adult passport renewals can be sent in by mail. To see if you meet the requirements, please review the Department of State's requirements here

Where can I obtain a certified Birth Certificate? 
Certified Birth Certificates can be ordered online at VitalCheck.com or through the Los Angeles County Register (births within the county). 

How do I obtain a passport within 2 weeks?
To obtain a passport within two weeks one must contact the Los Angeles Passport Agency Office at 1-877-487-2778 to make an appointment.
